Services

When the client needs one or more of the following care management services: 

• Community reintegration through residential, vocational and educational placement. 
• Medical, behavioral, substance abuse or rehabilitation services. 
• Assistance with understanding and following medical orders to aid in recovery.
• Coordination of services, including medical and rehabilitation appointments.
• Personal support, based on the client's needs.

And the client meets both of the following criteria: 

• Has a funding source for services needed.
• The client and/or guardian agrees to our services.

 The scope of work is to implement a care management model to best serve the client and their goals.
