 Care management is a team based, patient-centered approach to care coordination. It is designed to assist clients and their support systems in managing their current needs more effectively. 

 Some insurances are accepted, but anything not covered will be billed an hourly rate with a retainer fee. 

 We create an individualized treatment plan, including personal interactions tailored to your specific needs. 

 Services can be modified or stopped as needed.
