Our Care Managers follow HIPAA standards to ensure confidentiality. PCS terms and conditions will be reviewed before services begin.
Care Management Plan
Care management plans are created based on the following:
- Services identified.
- Diagnoses and personal information.
- Approval of custom plan by the client, family and treatment team.
- Care plan adjustments made based on changes in client’s needs.
Contract Type: PCS needs to know how to bill for services: Will this be done through the client, their family, an organization or private insurance, such as Blue Cross, Medicare, or Medicaid?
