About Us

Planned Care Solutions, LLC (PCS) was created as an independent, progressive care management company committed to reducing the risks and costs associated with chronic conditions. Our experience spans over 25 years of working as Care Managers for individuals with traumatic brain or spinal cord injuries, as well as varying degrees of mental and physical incapacities. We understand the need for individualized care management to achieve one’s maximum health potential. 

 Services include community reintegration through educational and vocational placements. We help our clients understand and follow medical recommendations and treatments. We also provide other support specific to the client’s needs. A portable medical profile is used throughout our relationship and then stays with the client, ensuring continuity of care. Our primary goal is to serve as a liaison between the treatment team, the family/loved ones and the client. 

Our objective is to provide progressive care management to our clients. Individualized care management plans are created to meet each client's needs. PCS works cohesively with the client, family/support group and the treatment team to assist them in reaching their goals. With our knowledge and forward-thinking philosophy, we  successfully work with challenging situations. Our Care Managers are dedicated professionals who assist clients on their path to better health.
